Near miss in the pattern

By NASA · March 29, 2022 ·

ASRS Report Human Factors

As we accelerated for takeoff, we see the Caravan coming straight towards us from the departure end of the runway. Right as we lift off, I make a left turn to side step the runway and we see the Caravan pass by right underneath us.

Student’s flight ends with raging fire

By NASA · March 24, 2022 ·

I saw a raging fire from the engine. I grabbed the fire extinguisher from under the co-pilot seat and tried to put it out but that did not work so I sprinted to the terminal to get the airport manager. He came out to help put the fire out.

By NASA · March 17, 2022 ·

By NASA · March 10, 2022 ·

The tower at KFNL is temporary and has no current access to radar but relies on visual observation from the ground level. Though radar has been installed, it has not been certified for almost a year now due to FAA-imposed Covid restrictions.

ATC calls get mixed up during training flight

By NASA · March 8, 2022 ·

ASRS Report Human Factors

After we landed we listened to the live ATC recording of our flight and came to the conclusion that ATC misspoke and that the call to “make left traffic pattern at your discretion” was meant for a different aircraft.

A stupid mistake I would never make…

By NASA · March 3, 2022 ·

ASRS Report Human Factors

I attempted to takeoff from the wrong runway at Charles M Schulz Sonoma County Airport (KSTS), at a known hot spot (Taxiway Hotel at Runway 20). This is a stupid mistake that “I would never make,” nonetheless, even after I made the mistake, I was not convinced that it happened, so much did I believe that I had lined up correctly.

Wiring issue cuts power to aircraft

By NASA · March 1, 2022 ·

After a difficult flight and a complicated landing, to think that negotiating the ground would be the most difficult part was stressful.
