CFI’s foot gets entangled in headset cord
During the turn, my foot became entangled in my headset cord. In an effort to manage the aircraft, I pulled my headset plugs from the ports and dislodged my headset on my head. It is a very confined area in a Cessna 152 and the ports are under the center portion of the instrument panel.

Improper fuel management leads to fuel starvation
The pilot’s improper fuel management, which resulted in fuel starvation and a total loss of engine power, and his subsequent improper landing flare, which resulted in a hard, bounced landing. Contributing to the accident were the pilot’s improper preflight inspection during which he failed to see that the right fuel cap was not secured.
