We were wrong on all accounts and it is a good thing that we were able to establish ATC contact before taking off, which would have only complicated things for all.

Advice to CFIs: Hush
CFIs often act as if they aren’t teaching if they aren’t talking. A more productive course of action might be to put greater emphasis on the pre-flight brief and the post-flight debrief. In between, allow the student the space to absorb the situation, analyze their own performance, and fix their own problems.

The pilot’s failure to install the engine oil filler cap before the first flight of the day; his subsequent failure to conduct a preflight check before the accident flight, which resulted in his failure to note that the cap was not installed; and his improper landing flare during a precautionary landing, which resulted in a bounced landing and ground loop.
