Builder Error Proves Fatal for RV-12 Pilot

Probable Cause: The pilot’s improper installation of the control stick pushrod assemblies, which resulted in separation of the left pushrod and a total loss of roll control during flight. Contributing to the accident was the failure to detect the installation error during the airplane’s construction, inspection, and subsequent maintenance.

Study finds equipment failures continue driving GA fatal accidents

While causing a small fraction of GA accidents, equipment failures continue to be fatal for pilots, even while other causes have declined, according to a new study. Even more disturbing: Many of the engine failures happened well before TBO and in parts that aren’t checked during an annual inspection.

Post-maintenance peril

New research, which examined more than 20,000 GA accidents, shows a 33.8% higher risk in the first hour of flight following an inspection. Even after that critical first hour, the risk remains elevated for at least 31 hours, according to researchers.
